Making it Simpler

Aged care navigation for families in Southeast Queensland

Helping older Australians remain safe, healthy and independent at home for as long as possible. We can help families prepare for the aged care assessment process, understand Support at Home and private care options, and explore allied health, strength, mobility and nutrition supports.

Giving family carers time to rest while helping their family member remain supported and socially connected. We can help family carers understand day respite, short-term residential respite and other temporary care options.

When staying at home may no longer be the safest or most practical option, we can help families understand the assessment process, explore residential aged care choices and plan the transition with greater confidence.

The aged care system can be difficult to understand. We help families navigate My Aged Care, understand available government-funded support and work out what steps to take next.

We help families prepare for the My Aged Care assessment process, identify the needs and concerns that should be raised, and understand the assessment outcome and support plan.

Our focus is helping older family members remain safe, independent and at home for as long as possible. We help families explore the combination of government-funded and private services that can make staying at home possible.

Feeling overwhelmed? We are here to help guide you.

We take the time to listen. We explain the options clearly. We help you plan the next steps. And as your family member’s needs change, we change with you.

Based in Brisbane, Wise Directions also supports families across Logan, the Gold Coast and the Sunshine Coast.

Available in person and online across Southeast Queensland.

Guidance you can trust

We work for you and your family — not for aged care providers. We do not receive commissions or referral fees, so our guidance is focused on helping you understand your options and make decisions that are right for your older Australians.

Aged care decisions are much easier when families have time to prepare. We can help you understand likely care needs, organise assessments and support, and put practical plans in place before circumstances become urgent.

We help families explore practical support that can make day-to-day life at home easier and safer, including meal preparation, shopping, house cleaning, gardening, showering and personal care, social support, strength and mobility programs, and nutrition advice.

We help explore physiotherapy and exercise programs, mobility equipment, assistive technology and home modifications that may improve safety and independence at home.

Carers can sometimes feel overwhelmed. We help families explore practical ways to share the caring load, including respite and day programs, social support, overnight assistance, short-term care and private nursing options.
